Form Level Settings

Form level settings menu can be located on the upper left side as seen below. 

Nam/Type details can be found under Designer Resources > Start Building a Form > Form Level Settings - Name/Type.

Admin Notification details can be found under Designer Resources > Start Building a Form > Form Level Settings - Admin Notifications.

The Other remaining settings details can be found under Designer Resources > Start Building a Form > Form Level Settings - Other.
