How do I access or view the Forms that have been Rejected (Returned for Revision)?

  • Click on the number found under the Reject (Return for Revision) queue. This will display all forms that have been Rejected (Returned for Revision) and not yet updated and resubmitted by the Form Participant. 

How do I access or view the Forms currently in progress that require further participants to complete their sections for Multi Participant Forms?

  • Click on the number found under the Multi queue. This will display all forms that have been completed by at least 1 participant but still need other Participants to complete their sections before the form is considered finalized. Once completed by all Participants, the form will be found under the Pending queue. 

How do I update the log out re-direct website for my Organization?

  • Contact DynamicForms.ITS@yale.edu and inform us of your Dynamic Forms Organization and the log out URL you would like participants redirected to after logging out of your Dynamic Forms Organization.

How do I update the Reject (Return for Revision) From Email Address for my Organization?

  • Contact DynamicForms.ITS@yale.edu and inform us of your Dynamic Forms Organziation and the email address you would like the Reject (Return for Revision) emails to come from.

How do I update the Reply-To Email Address for my Organization?

  • Contact DynamicForms.ITS@yale.edu and inform us of your Dynamic Forms Organziation and the email address you would like your Forms related emails to come from.

How do I give other people access to view/manage my Forms submissions?

Can I gain Designer permissions, so I can edit my Form?

Can I update my Organization’s Name?

  • Yes, Contact DynamicForms.ITS@yale.edu and inform us of your existing Organization’s name and what you would like your Organization name updated to.
