Form URLs

Form URLs for Participant to access

Dynamic Forms offers 2 types of URL’s that can be made accessible for participants to access your form. You can choose to use both URLs or one of the two, depending on your preference. Below are the Form URL options and their details.

Single Sign-On URL

SSO URL – If all participants have a NetID we suggest you use the internal (to Yale) URL that requires a participant to CAS in (log in using Yale’s Central Authentication Services) before being directed to your form. This allows for another level of security and validates the participant accessing your form.

e-Signature – This enables the ability to include an e-signature on your form. Since the participant CASed into the form, they can be validated so we know who is e-signing the form.

Save Progress/Draft Forms – This enables the ability for a participant to save their form progress at any time while completing the form. The participant can log out and log in at a later time with access to their Pending or Draft Forms. 
NOTE: Must enable Save progress feature under Form Settings - Name/Type.

Forms History – This enables the ability for a participant to view and print any forms submitted in the past or view any forms currently in progress.

Non-Single Sign On URL 

Non-SSO URL – If participants may not/will not have a NetID we suggest you use the non-SSO URL for external (to Yale) participants which does not require a participant to CAS in before being directed to your form, but may require the participant to create a Dynamic Forms account depending on your forms configuration, see below for details.

If no signature is required on your form, the participant can access the URL and be directed to complete your form.

If a signature is required on your form, the participant can access the URL which will require them to create a Dynamic Forms User Profile before being directed to complete your form. This is due to the requirements around using a signature within a form and needing further information to validate the participant signing the form is who they state they are.

Save Progress/Draft Forms - By creating a Dynamic Forms User Profile the participant will have the ability to ‘Save Progress’ at any time while completing the form. The participant can log out and log in at a later time with access to their Pending or Draft Forms found under My Forms > Pending / Draft Forms.
NOTE: ‘Save Progress’ only works if its setting has been enabled (under Form Settings Menu - Name/Type - Allow Drafts to be Saved = Yes) and participant has created/logged in to their Dynamic Forms account. If no Dynamic Forms log in was created, they will not have the ability to ‘Save Progress’. 

Forms History – By creating a Dynamic Forms User Profile the participant will have the ability to access their Forms History. This enables the participant to view and print any forms submitted in the past under their User Profile.

Your Forms URLs

Your form will have both a SSO URL and a Non-SSO URL available.

1. Find your Form and go to Action drop down list. 

2. Select URL Instructions

3. The URL Instructions window will appear that contains both URLs for Non-SSO Forms and SSO Forms. Copy the URL(s) that works best with your form.
